Form Information: This form must be submitted by the authorised school representative (e.g HOD or VET Coordinator).
One form submission is required for each pre-screened student.
Before you begin, ensure that you have:
- received, reviewed and retained a completed copy of the ADI parent consent form and Student EOI Form for each candidate (to be uploaded during the submission).
- Discussed any initial concerns with the parent and student relating to consent or suitability from the school perspective. (e.g: misalignment with interests, conflicting schedules, key eligibility criteria)
- Checked available school systems for suitability and eligibility evidence requirements.
